Thank you for choosing Rukai Botanicals. This questionnaire helps us create a blend of product(s) that suits you and, just as importantly, that is safe for you. It takes about 10–15 minutes.

Please answer as fully as you're comfortable with. Questions marked * are required. Everything you share is kept confidential.

Some herbs act on hormones, so this helps me choose safely.

These questions are essential. Some herbs interact with medicines or aren't suitable for certain conditions, so please be as complete as you can.

Include the contraceptive pill, HRT, antidepressants, blood thinners, blood pressure, diabetes or thyroid medication, and painkillers. Even those you've stopped taking in the last year. Write "None" if not.

Please include food, plant, pollen and hay fever allergies (especially to daisy-family plants such as ragweed, chrysanthemum or chamomile). Write "None" if not.
